The following forms are available online.

Restitution Request Form
This form is used for tracking and calculating restitution amounts associated with criminal cases in which economic loss occurs. 

Non-Prosecution Request Form
This form is used when a victim of a reported case no longer wishes that charges be pressed. Whether or not charges are filed is ultimately at the discretion of the prosecutor; however, this form may be used to document the victim's request and will be added to the case file.

Civil Harassment Restraining Order
This form is used to request a civil (non-criminal) restraining order. The District Attorney's Office provides this form but is not involved in the filing process. For filing and questions, applicants may contact the Mono County Superior Court at (760)934-5444.

Domestic Violence Civil Restraining Order 
This form is used to request a civil domestic violence restraining order. The District Attorney's Office provides this form but is not involved in the filing process. For filing and questions, applicants may contact the Mono County Superior Court at (760)934-5444.
